Assessment process
Our assessments are conducted in the individual’s home for accuracy and clarity of required care needs. The assessment process includes:
- Face-to-face interview: personal interaction to understand the individual’s needs
- Clinical observations: detailed observations of the individual’s function and impact of condition/injury
- Formal assessments: where required or appropriate, standardised tests to evaluate capabilities
- Review of collateral information: examination of reports and medical documentation
- Collateral interviews: discussions with allied health professionals, care agencies and those supporting the individual
